Jobsite Rules Acknowledgement

Please read the following and complete the acknowledgment form at the end.

  1. ANSI approved hard hats, safety glasses and retroreflective vest shall be worn by all Greystone contractors and visitors. 
  2. Hard soled work boots (no tennis shoes), long pants and a shirt are required.  
  3. Face shields AND safety glasses must be worn during grinding, chipping, jack hammering or other activity which creates flying debris or hazard. 
  4. Hand/hearing/respiratory protection as required. 
  5. Fall Protection measures will be utilized at unprotected heights over 6 feet. A Fall Protection Plan must be submitted and approved prior to the work.  
  6. Ladders are to be used in accordance with manufacturer guidelines on a level surface. 3 points of contact must be maintained when ascending or descending ladders. Extension ladders are to be secured and must extend at least 3’ above the landing surface. 
  7. Clean and organize your work area daily. Passageways and stairs need to always be clear of objects. 
  8. No smoking, no spitting tobacco or seeds. 
  9. Only authorized personnel are permitted to operate equipment. Daily inspections are required. Seat belt use is mandatory, when equipped. Cell phone use is prohibited when operating machinery. 
  10. The use of intoxicants, weapons, harassment, or unsafe actions will not be tolerated. 
  11. All incidents (injuries, damage to property, or unsafe conditions) will be reported to the Greystone jobsite supervisor immediately.   
  12. Know the locations of fire extinguishers, first aid kits, evacuation routes and gathering points. 
  13. Flammable liquids must be kept in approved, metal containers. 
  14. Trade partners are responsible for the safety and securing of their own tools and materials.  
  15. No use of audio producing earbuds or headphones while engaging in work will be allowed. 
  16. Holes and floor openings larger than 2” must be covered or protected with guardrails. Covers must be labeled “Hole” and secured against displacement. ​
  17. Prior to any structural erection or critical lift, a plan must be submitted and approved prior to the work. 
  18. Scaffolds must be erected and inspected daily by a competent person. Tags indicating the scaffold’s status must be present. Ladders must be attached or built into the frame. Railings must be used at unprotected heights over 6 feet. 
  19. Hot Work Permits are required each work shift and prior to any activity which may produce a spark or flame near combustible areas. 
  20. Power tools shall be kept in good working condition. Guards shall not be removed. Handles must be present if the tool requires it for use. 
  21. Extension cords shall be 3-wire heavy duty round types free of damage with ground prong intact. GFCI (Ground Fault Circuit Interrupters) must always be used. Immediately remove damaged cords from jobsite. 
  22. Toolbox Talks required weekly by all workers on the jobsite.

  24. Any contractor disturbing the grade, must perform their own State Utility Locate One Call. Private locates required as applicable. 
  25. Before working in an excavation, a competent person must inspect and ensure that it is safe for human occupancy. 
  26. A Silica Dust Exposure Control Plan is required when cutting, grinding, sanding or sweeping affected materials. 
  27. Temporary lights shall have protective covers and shall not be suspended from cords unless specifically designed for suspension. Electrical switch and cover plates shall never be removed on live circuits. 
  28. Proper lockout/tagout procedures should be implemented prior to any activity where electrical, mechanical energy or stored energy hazards are present. 
  29. Monitor daily and document carbon monoxide levels when using combustible engines/heaters in enclosed spaces. 
  30. Prior to any confined space entry, a plan must be submitted and approved prior to the work. Entrants and attendants must be properly trained.
